Job Description

The Nurse Navigator is an essential component of the Curative team responsible for coordinating complex medical care issues identified during the baseline visit. This role involves providing guidance on medication related questions, preventative screenings, and ensuring comprehensive care coordination. The Nurse Navigator will play a key role in improving patient outcomes by providing needed follow-up in areas identified during a member's Baseline Visit

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Care Coordination:

• Coordinate complex care issues identified during baseline visits, ensuring timely follow-up and intervention.
• Schedule patients' appointments, tests, and procedures as needed.
• Consult with the assigned provider or supervising physician regarding complex patients with urgent needs.

Medication Management:

• Assist members in understanding Curative medication formulary and tiers
• Discuss medications with patients, providing detailed information on usage, side effects, and interactions, referring to a clinical pharmacist as needed

Preventative Screenings:

• Follow up on recommended preventative screenings, ensuring patients complete necessary tests and procedures.
• Provide education on the importance of preventative care and lifestyle habits, including nutrition and physical activity.

Patient Support and Education:

REQUIREMENTS:

Current unencumbered active license to practice as a Registered Nurse in Texas.

Must be detail-oriented and able to multitask effectively.

Excellent communication and written skills.

Ability to work independently and as part of a collaborative environment.

Comfortable with learning and using new technology and software.

PREFERRED EXPERIENCE/SKILLS:

Experience as a Nurse Navigator or Case Manager.

Experience working with telehealth services.

Bilingual (Spanish)

EXPECTED HOURS OF WORK:

8am to 5pm (some flexibility available as determined by the needs of the clinic).
